Sacred Items – These start dropping in World Tier 3 (Nightmare) and represent a step up in item power from regular gear. Sacred items offer stronger base stats and higher affix rolls, making them essential for late-game content.
Ancestral Items – Available only in World Tier 4 (Torment), Ancestral items are the pinnacle of gear in Diablo 4. They offer the best possible affix ranges and are required to push the most difficult content. Many top-end Diablo IV trading deals involve Ancestral Legendaries and Uniques.
